Janowski Insurance Services is committed to protecting your privacy. This Privacy Policy explains what information we collect about you, how we use it, when we share it, and what choices and rights you have. It applies to information we collect through our website (janowskiinsuranceservices.com), through phone calls and emails with our office, and during the insurance application and enrollment process.

1. Information We Collect

Information you give us directly

When you contact us through this site, request information, or apply for coverage, you may provide us with:

  • Name, address, phone number, email address, and date of birth
  • Information about your current insurance coverage and household
  • Income and household-size information used to determine eligibility for ACA premium tax credits
  • Health and medical history information needed for life insurance underwriting
  • Medicare beneficiary information (Medicare number, Part A/B effective dates) for Medicare enrollment
  • Government identifiers (such as Social Security number) when required for an insurance application
  • Bank account or payment information when needed to set up premium payments

Information collected automatically

When you visit this site, we (and the analytics services we use) automatically collect:

  • IP address, browser type and version, operating system, and device type
  • Pages you visit on our site, time spent on each page, and the website that referred you
  • Cookies and similar technologies used to remember preferences and measure site usage

2. How We Use Your Information

We use the information we collect to:

  • Respond to your questions and quote, compare, and apply for insurance products on your behalf
  • Submit applications to insurance carriers and complete enrollments
  • Provide ongoing service, including renewals, plan reviews, and Special Enrollment Period changes
  • Communicate with you about your coverage, plan changes, enrollment deadlines, and Marketplace tax forms (such as Form 1095-A)
  • Comply with applicable federal and state insurance laws and regulations
  • Detect and prevent fraud, security incidents, and unauthorized use
  • Improve our website and the services we provide

3. How We Share Your Information

We share your information only with:

  • Insurance carriers and Marketplaces. We share the information necessary to obtain quotes, submit applications, and complete enrollments. This includes HealthCare.gov for ACA Marketplace enrollments and the issuing carrier for Medicare and life insurance applications.
  • Service providers. Third parties that help us operate our business — for example, customer relationship management software, electronic-signature services, and email service providers — under written agreements that limit how they may use your information.
  • Government agencies and regulators. When required to comply with applicable law (for example, IRS reporting under the Affordable Care Act, CMS audits, or state Department of Insurance examinations).
  • Other parties with your consent. If you ask us to coordinate with a tax professional, financial advisor, or family member, we will share information only as you direct.

We do not sell your personal information for marketing purposes, and we do not share it with third parties for their independent marketing.

4. Privacy Notices Required by Federal Law

Gramm-Leach-Bliley Act (GLBA) notice

Under the Gramm-Leach-Bliley Act and its implementing regulations, financial institutions including independent insurance producers must explain how they share nonpublic personal information. Our practices are summarized in the table below. You have the right to opt out of any sharing for which a federal opt-out right applies; we currently do not engage in any such sharing.

HIPAA

When we handle health information on behalf of an insurance carrier, we do so subject to the Health Insurance Portability and Accountability Act (HIPAA) and any business-associate agreement we have with that carrier. We maintain administrative, physical, and technical safeguards designed to protect your protected health information.

5. Data Security

We maintain reasonable administrative, technical, and physical safeguards designed to protect personal information against unauthorized access, use, alteration, or destruction. These include encrypted storage and transmission of sensitive data, access controls limited to authorized personnel, secure handling of paper records, and ongoing review of our security practices.

No method of transmission or storage is perfectly secure. If we become aware of a security incident affecting your personal information, we will notify you and applicable regulators as required by law.

6. Your Rights and Choices

Depending on where you live, you may have the right to:

  • Request a copy of the personal information we hold about you
  • Request that we correct inaccurate information
  • Request that we delete your information, subject to legal recordkeeping obligations
  • Opt out of marketing communications (you may continue to receive non-marketing service messages about your coverage)
  • File a complaint with a state insurance department or other regulatory authority

Virginia Consumer Data Protection Act (VCDPA)

If you are a Virginia resident, the VCDPA gives you the right to confirm whether we process your personal data, access it, correct inaccuracies, request deletion, obtain a copy of your data, and opt out of certain processing. To exercise these rights, contact us using the information at the bottom of this page.

California Consumer Privacy Act (CCPA)

If you are a California resident, you have the right to know what personal information we collect about you, to request that we delete your personal information, to opt out of any sale of your personal information (we do not sell personal information), and to non-discrimination for exercising your rights.

Other states

Several other states have enacted consumer-privacy laws with rights similar to those described above. If you live in a state with such a law, you may exercise the rights it provides by contacting us. We will respond as required by the applicable law.

7. Cookies and Online Tracking

Our website uses cookies and similar technologies to remember your preferences, measure traffic, and improve site performance. You can control cookie behavior through your browser settings, including blocking or deleting cookies; doing so may affect the functionality of some site features.

Our services are intended for individuals 18 and older. We do not knowingly collect personal information from children under 18. If you believe a child under 18 has provided us with personal information, please contact us and we will take steps to delete it.

10. Changes to This Privacy Policy

We may update this Privacy Policy from time to time to reflect changes in our practices, the law, or the products we offer. When we make material changes, we will update the "Last Updated" date at the top of this page and, where required, notify you directly. We encourage you to review this Privacy Policy periodically.
